5. DOKUMEN LENGKAP (KTP, KK,FOTO 4X6 ( 4 LEMBAR) , FOTO 3,5X4,5 (4 LEMBAR) UKURAN MUKA 80% ( UNTUK VISA) ,AKTE ,IJAZAH , BUKU NIKAH BAGI YANG SUDAH MENIKAH ,PASPOR , SERTIFIKAT VAKSIN).
